You can send scanned copies of these forms back to us through MyDocsOnline.com or you can send us your completed documents using secure faxing (630)529-0137 or you can bring them in with you.
The faxes that come into our office come from a secure facility which encrypts the data and sends it to our office. Then it needs to be unencrypted by our staff. They are HIPPA compliant. In this way we can ensure your privacy. They do not lay around in the fax machine which is unattended and they are secure an encrypted.
